While not yet available in pharmacies, Onswik (insulin efsitora alfa-gobe) injection for subcutaneous use was approved by FDA on September 23, 2026. So, what’s new? This is a once weekly injectable insulin (instructed to be injected on the same day of the week), which according to the Lilly webpage here (and also by my own calculations) reduces the number of injections that a patient has to administer to themselves from 365 to 52!

If it were me and I needed multiple injections each week, having the option to reduce that to just one injection a week would definitely make my life a little easier!

“Onswik is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus”. The label notes the following limitation “Onswik is not recommended for use in patients with type 1 diabetes mellitus due to increased risk of severe hypoglycemia”. The product comes in a KwikPen for ease of administration and should only be used in the container closure system in which it is dispensed. The prescribing information (here) and the website indicated that patients must be trained in the use of the product and there are numerous warnings regarding the proper use of the product as well as the device in which it comes. Regardless of the need for training, this new product greatly reduces the burden on type 2 patients for whom if may be appropriate.
